"There are no 'tokens' it is an actual item. So say you have a tanking plate chest piece drop from one of the bosses in sunwell, you take that and you turn it in with a Sunmote, swapping it for a dps chest piece. If you mouse over the 'token icon' you see the offset item it wants in exchange.
Edit: An example to make it more clear, in the picture below you can see him hovering over the 'token icon' which brings up the icon for the Pauldrons of Perseverance(tanking shoulders) the icon is listed as one of the items needed to purchase the Pauldrons of Beserking(dps shoulders)."
